Care Plus Group provides a 24-hour telephone triage service, the Clinical Assessment Service (CAS)
This service is staffed by trained clinicians, nurses and paramedics, with knowledge of services available in North East Lincolnshire who can give health advice to patients or access to GP out-of-hours appointments for urgent health problems.
Our clinicians have varied healthcare experience and can give a wide range of advice and signpost to other services as appropriate.
The service also uses a clinical decision software programme, as an aid to triaging patients; this software is evidence-based and uses the latest information.
GP out-of-hours calls are triaged, and appointments for acute, urgent issues can be offered. Please note GP Out of Hours cannot see patients for ongoing issues or offer reviews or second opinions.
CAS can escalate to our Community Urgent Care Team if needed (CUCT)
The falls team service can also be accessed through CAS for fallers at home, who are not injured. The team can visit, assess and assist up using specialist equipment if required.
