K had previously lived independently and brought up children but had been living with her parents for 18 months and they had been supporting with all activities of daily living and personal care tasks which was putting a strain on their relationships.
When completing the initial assessment, it became evident that K was struggling with much more than bathing. K struggled with memory and had lost a lot of independent skills and was relying on her parents.
She was supported to the falls group to improve strength and balance and integrate into the community and supported to health appointments and worked with a Learning Disability nurse to access a CT scan which showed a diagnosis of Alzheimer’s.
With support from Focus, a suitable property was sourced which met K’s social and environmental needs which K has now moved into. K now accesses several activities in the week such as the discos, health walk and day services.
K is very happy where she is living and appears to be thriving. Mum and dad are also very happy with the service and input from services and this has improved their relationship and that with their daughter.
