The Community Nursing service provide home, residential care or community based nursing care, treatment and support to people registered with a GP practice in North East Lincolnshire who are housebound or have severe mobility problems which means they are unable to attend the surgery for treatment.

Our teams work in partnership with the local GP Primary Care Network’s (PCN’s) delivering care and support to people requiring on-going nursing care. They are based within several of the GP Practices throughout North East Lincolnshire.
The teams include Nurse Managers, Clinical Leads, Community Advanced Practitioners, Nurse Team Leaders, Nurse Associates, Associate Practitioners, Community Nurses, Health Care Assistants and Phlebotomists and provide a range of nursing interventions including long term condition management, wound management, continence care, injections and end of life care.
To provide nursing care, interventions and support to individuals wherever they reside who through ill-health or mobility problems are not able to access treatment in their GP practice, ensuring patients and their families/carers are supported to remain as well and as independent as possible, enabling them to have the best quality of life. We provide high quality nursing interventions, support and care to our patients and carers during what can be a very difficult time due to poor health.
We understand that you may be anxious about letting someone into your home. All our community nurses will be wearing a Care Plus Group uniform and carrying ID which you can request to see at any time. As a service you may receive a visit from a member of staff who is accompanied by a student nurse or a trainee who is undertaking training or an apprenticeship, this is in order to facilitate their training and development. If a member of staff cannot show you a Care Plus badge, please do not allow them into your home.
The Community Nursing team work in partnership with patients and their families/carers and the wider multi-disciplinary team to provide person-centred, coordinated care which enables people to make informed decisions regarding their care that is right for them.
There will be an initial visit and assessment and following on from that a personal care plan will be drawn up based on your nursing care needs. The nurse will go through this with you and you will be asked to consent to it.

Out of hours you can contact Single Point of Access on 01472 256 256, which is staffed 24 hours a day, 7 days a week and is subject to an assessment by a health or social care professional.
